Completed all modules but not received certificate

I have completed my corse but my certificate is not issued.

I have got certificates after completion of courses. But my certificates are not available in the 'Learner passbook' or 'My learning' sections. It was there after completion of course but it is disappeared itself by course of time. Please help to recover my lost certificates Thanks
